8.) Writing Intensive/Writing in the Disciplines: Be sure to know your catalog year in order to determine which requirement applies to you. There are some courses that were previously on the "Writing Intensive/Writing in the Disciplines" list that are no longer requirements, based on catalog year. if your catalog year is effective Fall 2007 and thereafter, click here. If your catalog year is previous to Fall 2007, click here.

9.) Requesting a Major or Minor Degree Plans & Petitions: These documents are essential for completing your major or minor in Math, as well as, may be required for graduation. If you are majoring in Math, there must be a major degree plan on file before you graduate. You should request this via a Degree Plan Request. If you are minoring in Math, you must provide a copy of this minor to the department of your major in order to graduate. This also applies if you are minoring in another subject and are a Math Major. Your signature is required after these forms are signed by the department and by the college. You should be contacted via e-mail or postal mail. After signing the document, you should be provided with a copy for your records.

General Petitions are necessary if you have transfer credit that didn't transfer as its UH equivalent. After these forms are signed by the department and by the college, you should receive a copy of the petition in the mail.

If you also plan to do the following, you should complete a petition:

a.) Change admission status

b.) Change of classification (freshman, sophomore, junior, senior)

c.) Request course overload

d.) Change of degree objective

e.) Degree requirement exception

f.) Change of major

g.) Request a special problems course

h.) Request a course equivalency

Please submit all of these forms to 120 Old Science (UAC). The processing times for these documents vary. It should take no longer than 2 weeks to have them processed.
